This form can be used to withdraw an application or referral you have made to the Queensland Civil and Administrative Tribunal (QCAT).

To complete this form you will need to provide:

      Some types of applications require leave of the tribunal to withdraw an application or referral. To apply to withdraw your application in these jurisdictions, complete and lodge a Form 40–Application for miscellaneous matters.
