Use this form to apply for QCAT’s permission to be represented in a proceeding.

You can only apply if you are an applicant, respondent or an active party in a QCAT matter.

To complete this form you’ll need:

  • your case number
  • the respondent’s name (if applicable)
  • your representative’s contact details.

Entities wanting QCAT’s permission to be represented by a lawyer or government legal officer should not fill in this form. Instead, you should write to QCAT setting out the details of your matter and giving reasons why QCAT should let someone else appear for you.
