Before you apply to review a decision, you should check you are eligible to apply.


2. Complete and lodge this form

To make an application for a review of a voluntary assisted dying decision, complete and lodge the application form below. Carefully read the form instructions, answer each question and tick an appropriate box when prompted.

  • copies of all relevant documents are included

The review may be delayed otherwise.

3. How to lodge your application

4. What happens next?

When your application is made:

  • no further steps in the voluntary assisted dying process may be taken. The request and assessment processes is suspended until the application for review is finalised
  • you will be provided with a case number and advised about the next steps
  • QCAT will provide you and any other party with a copy of the application and any supporting material. You do not need to provide this information to any other party.
