To make an application in relation to an occupational and business regulation matter complete and lodge the appropriate form.

Disciplinary proceeding

This form is used by a board, chief executive, commission or other entity to make an application or refer a disciplinary proceeding to QCAT.

Review of a decision

Use this form to apply to review a decision made by an entity (for example a government department or government board) empowered to make that decision under Queensland legislation.
