Application process - children and young people matters
To make an application in relation to a children or young people matter complete and lodge the appropriate form.
Remember to attach a copy of the letter from the department outlining the original decision to your application form. If you don’t have a copy of the letter contact the department to get one.
Review of a decision - blue cards
Use this form to apply to review a decision of a state agency (including a decision about a blue card).
Review of a decision – childrens matters
Use this form to apply to review a decision made by Department of Communities, Child Safety and Disability Services (Child Safety Services).
Exclusion from school grounds
Use this form to apply to prohibit a person from entering school grounds.
Notice of election
Use this form to request to become a party to a review of a matter under the Child Protection Act 1999.
