Application process - children and young people matters

To make an application in relation to a children or young people matter complete and lodge the appropriate form.

Remember to attach a copy of the letter from the department outlining the original decision to your application form. If you don’t have a copy of the letter contact the department to get one.

Review of a decision - blue cards

Use this form to apply to review a decision of a state agency (including a decision about a blue card).
