Application process - children and young people matters

To make an application in relation to a children or young people matter complete and lodge:

Form 17 - Application to review a decision - childrens matters (PDF, 482.0 KB)

Form 18 - Child Guardian application (PDF, 328.7 KB)

Form 19 - Application for prohibition on entering premises of instructional institutions (PDF, 383.0 KB)

Form 23 - Application to review a decision (PDF, 403.4 KB)

Notice of election (PDF, 309.6 KB)

Attach a copy of the letter from the department to your application form. If you don’t have a copy of the letter contact the department to get one.

Once your application and forms have been received QCAT will review the information you provided. You will then be contacted by QCAT about the next steps that will happen in relation to your case.
